We work hand in hand with GPs, allied health professionals, schools, and support coordinators to make sure our services sit within a well-informed, team-based model. From sharing updates for plan reviews to attending multidisciplinary meetings, we are committed to ensuring that everyone who supports the participant is on the same page and working toward the same goals.
We prepare written summaries, update letters, risk alerts, and case notes in plain English to ensure all key people can access and use the information that matters most.
